The Oregon Health Plan (OHP) is Oregon’s version of the federal health care program called Medicaid.

Enrolling in OHP does not have a deadline.

Our Health Navigators (541-766-2130) can assist you with this process if you have questions or difficulties with your application or renewal.

How to use your OHP coverage

If you use the Oregon Health Plan (OHP), Oregon’s version of Medicaid, you don’t have to wait to see a doctor. Call us once you have applied and we’ll get you an appointment while your OHP application is processed.

If you are newly enrolled in OHP, we encourage you to schedule an appointment with our Nurse Care Coordinator before your first visit (we call this an ‘establish care visit’).  At the ‘establish care visit’ you will review your health history, the nurse will screen for health risks, and you will establish your health record at our clinic.  Once you have completed this process, same-day or next-day appointments are available to address immediate concerns.

What is a Coordinated Care Organization (CCO) and what can it do for me? 

If you are on OHP, it is very likely you are part of a CCO. There are 16 CCOs in Oregon.

The CCO that covers Linn, Benton, and Lincoln Counties is called InterCommunity Health Network, IHN-CCO for short. If you live in Lane County you might also be covered by, or have the option for coverage from, Trillium CCO.

This video shares some of the benefits of CCO work. Those benefits include: communication between providers, coordination of your care, referrals to classes and other health services.
